O PROGRAMA DE TREINAMENTO E DESENVOLVIMENTO COOPEN, item indispensável nesse processo de adesão, será realizado presencialmente na sede da COOPEN para os residentes de Fortaleza e área metropolitana e de forma virtual para os residentes do interior.
Somente será expedido o Certificado para o profissional que participar na TOTALIDADE DO TREINAMENTO, de forma individual e com a câmera ligada (para participantes on-line)!

✷ Dados Bancário EXCLUSIVAMENTE do Bradesco/NEXT (conta corrente ou poupança) ou SICREDI (conta corrente)
✷ Cédula Profissional do Conselho Regional
✷ Certidão Negativa do Conselho Regional
✷ Comprovante de Formação Profissional na Área da Saúde (diploma)
✷ Comprovante de Experiência ou estágio extracurricular (mínimo de 06 meses)
✷ Atestado de Saúde elaborado por Médico (informando que goza, ou não, de plena saúde física e mental)
✷ Inscrição INSS/PIS/PASEP
Para emitir a numeração sem a Carteira de Trabalho Física:
1- Portal meu INSS: Acessar o site meu.inss.gov.br, fazer login e consultar o extrato de contribuições ou PIS.
2- App Meu INSS: Baixar o aplicativo, fazer login e consultar o PIS ou extrato de contribuições.
3- Caixa Econômica: Acessar o site caixa.gov.br ou liguar para 08007260207 solicitando informações sobre o PIS.
4- Ministério do Trabalho: Consultar diretamente no site do Ministério para obter o NIT, caso não tenha.
